Application Form for Joining Old Guard

There are 3 ways to submit an application form:
- Download the
[ docx version ]
of the form, fill it out electronically, and send it to your contact at Old Guard as an email attachment.
- Open the
[ PDF version ]
of the form, fill it out electronically, save it to your computer, and send that to your contact at Old Guard as an email attachment.

Open and print either of those versions (they're equivalent), fill it out by hand, and bring it to a meeting.
Or use a form from the supply kept at back during meetings.
NOTE: We have a preference for #1 filled-in docx file
if you are able to do it, since it makes data entry easy.
Next best is #2 filled-in PDF file,
if that works for you. But filling it out by hand and and bringing it to a meeting will
be just fine.
